This deliverable was requested by the European Commission during the Grant Agreement process in order to address Ethics Requirement 2, which refers to the ethics related to humans and data. The scope of the deliverable is to make available, since the beginning of the activities, the templates of the informed consent/assent forms and information sheets, that are needed to collect data and information from external participants. Such templates are designed fully compliant with EU Regulation 2016/679 on the protection of personal data (in force from 25 May 2018), repealing Directive 95/46/EC, on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (general data protection regulation - GDPR). From a methodological point of view, a focus group on "ethics requirements, data policy/management and on the relevance of GDPR to CarE-Service" was organized on 14th June 2018 during the project the Kick-off meeting, attended by all participants. In the focus group, the relevance/applicability of some personal data being collected non-anonymously through two project partners' platforms (PROD and EVAI) were foreseen. Based on a thorough study of relevant legislations (mainly GDPR and on the discussions in the focus group, templates were drafted, verified and commented by all partners and finally translated in to relevant languages. If necessary, this document will be updated according to the eventual need of improving or modifying the templates based on project activities. In particular, the project foresees the collection of data from consumers, but data collection is not designed yet. Thus, it is not known at the moment in which countries data collection will take place. If it will be decided to collect personal data from the citizens/companies in countries with local languages different from the ones already translated (German, French, Spanish and Italian), the new translations will be provided and this deliverable will be updated. The deliverable will be updated also if, in the course of the project, some amendments on the consent will be required due to the peculiarity of data collection processes that will be known at the time of the data collection.
