The communication between general practitioners and secondary hospital-based physicians very often presents deficits, which may negatively affect continuity of care. In this paper, we aim at improving this communication by means of a publish/subscribe service for the fast and efficient notification of medical data between different healthcare providers. We go beyond the state of the art of current notification architectures by introducing the concept of topic hierarchies.
