In this paper, we present an information system based on a social network architecture for the Inclusive Community, called SICS. The scope of proposing such a system is to promote social and health care integration, strengthen territorial networks, and facilitate communication among social workers, fragile people and resources (i.e., volunteers, etc.). The paper outlines the methodological considerations with regard to information management, modelling and design of data related to the SICS, and describes the related underlying data model. The implemented system is composed of a web based back-end and three Apps for, respectively, the assisted person, community social workers and volunteers, and messaging. SICS is designed and developed within the INSPIRE project and is used to achieve its required goals.
