The aim of this chapter is to give some insights about the potential of a simulation approach in a clinical setting with particular reference to patients in advanced heart failure with a view to predict outcome and optimise device treatment. CARDIOSIM© software is a suitable tool able to accurately generate haemodynamic parameters matching those measured invasively and non invasivaley and deliver reliable simulations in the presence of LVAD support. The circulatory equilibrium framework approach is also discussed in relation to its quantitative predictive value during mechanical circulatory support.
