Diabetes mellitus type 2 (DMT2) is a pathological condition with an important socio economic impact characterized by elevated blood glucose levels due to insulin resistance that involves several complications: the main manifestation is hyperglycemia, but an analysis of the literature shows also a correlation between DMT2 and bone health[1]. Patients affected by DMT2 show an increased risk of fractures, 5 times higher compared to healthy patients, even if bone mass and strength are in a normal range or increased, this phenomenon is called the diabetic paradox and this is the reason why it’ very important to focus the attention on the assessment of bone quality and quantity. The increased risk of fractures in DMT2 may be due to the glycosylation reaction that occurs between excess blood glucose and collagen; this interaction causes a conformational alteration of the bone structure, making it more fragile[2]. Another concomitant factor that negatively impacts the bone health status is obesity (a condition often associated with DMT2), for the inflammatory effect of cytokine released by adipose tissue[3].
