The project aims to create an interdisciplinary tool , flexible in its clinical and research use (clinical/epidemiological) to evaluate cardiovascular risk in an integrated way. The use of a computerized system will allow the physician to perform check ups based on the indications of international guidelines updated on the specific topic. This procedure will lead to a more accurate and complete diagnosis, which at the same time allows the patient to understand better his/her condition and the personalized aims to pursue. The objectives of the project are: uniform the behavior of the diverse specialistic branches involved (cardiology, diabetology, ) with that of the practitioner by means of common guidelines stored in a software program able to indicate the most appropriate integrated diagnostic/ therapeutic pathway; increase the appropriatness of the diagnostic-therapeutic procedures within the overall management of the patient with cardiovascular risk by stimulating the cultural exchange between the several specialistic branches and the in-and outpatient management; decrease the chance of error, distractions, omissions, especially in evaluating the cardiology patient as to secondary prevention; define a multiparametric map for evaluating cardiovascular risk, focussed on a local population and based also on new CV risk factors; By applying this system in routine practice is in the optical that "the public health system will have immediate access to the progresses in diagnosis and treatment of patients derived from scientific research into".
